Required Documentation for Registration
You must bring the following documents with your application in order to register:
First-Time Applicants
- Copies of your two most recent pay stubs, dated within 30 days of registration
- If you do not have your pay stubs, you will need to submit a letter from your employer listing your Social Security number, hourly pay rate, date of hire and weekly hours.
- If you are on Disability or Workers' Compensation, submit a copy of your last pay stub and your most recent disability stub. If you do not have your last pay stub, your disability stub must be accompanied by a letter from your employer verifying your employment, Social Security number, hourly pay rate, date of hire and weekly hours.
- Copy of your 1199SEIU Health Benefits ID Card, listing your dependent children. If your children are not listed on the 1199SEIU Health Benefits ID Card, other acceptable proof of legal dependency is required, such as:
- Adoption papers listing you as guardian
- Birth certificate listing you as parent
- A copy of your most recent signed tax return, listing child(ren)
If your bargaining unit does not contribute to the 1199SEIU National Benefit Fund, you may submit your participating 1199SEIU Health Benefits ID Card if your dependents are listed on it. If not, you must provide proof of legal dependency as listed above.
- Copy of the Social Security card of each child for whom you are applying
- If you do not have a copy of a child’s Social Security card, you must provide the Child Care Funds with:
- An original letter from the Social Security Administration, showing the child’s Social Security number
- A copy of your most recent signed federal tax return, showing the child’s Social Security number
If you do not have any of the above documents, the Child Care Funds will not be able to register your child(ren).
Returning Applicants
- Copy of your most recent pay stub, dated within 30 days of registration.
- If you do not have your pay stub, you will need to submit a letter from your employer listing your Social Security number, hourly pay rate, date of hire and weekly hours.
- If you are on Disability or Workers' Compensation, submit a copy of your last pay stub and your most recent disability stub. - Your 1199SEIU Health Benefits ID Card listing your dependent children must be presented to the registration clerk when you submit your application.
- If your children are not listed on the 1199SEIU Health Benefits ID Card, other acceptable proof of legal dependency is required, such as:

- Social Security Card: Applicants who have registered with the Child Care Funds (after 2000) will only need to bring in the Social Security cards for new children.
If you do not have any of the above documents, the Child Care Funds will not be able to register your child(ren).
