Eligibility Information

This benefit is available for the following funds:

  • National Benefit Fund
  • Greater NY Benefit Fund
  • Home Care Benefit Fund

Wellness ProgramThrough the Benefit Fund's free, convenient and confidential Wellness Program you receive screening and referrals for hypertension, high cholesterol, diabetes and weight management at more than 20 participating institutions across New York City. Through monitoring, education and support, the Wellness Program’s nurses will help you work with your physician to manage your condition and maintain your health.

The Wellness Program’s clinical services are provided by Worksite Medical Services P.C.
