The Fund has two types of providers: Panel providers and Member Choice providers.

To apply to be on our panel of participating providers, email us at providers@1199Funds.org or call our Provider Hotline at (646) 473-7160 for an application.

Member Choice providers must be affiliated with one of our participating Member Choice hospital networks. To check if your hospital is in the Member Choice network, search our directory.
