For Home Care Members: Important Information about Your Health Coverage! Take the Survey by July 17
We are asking all home care members to complete a brief survey about their household size and income to help us determine how we provide you with continued health coverage. Regardless of your household income, if you work 100 hours or more a month, you will be entitled to healthcare benefits, and it is expected you will not have to pay more than you currently pay.
Please use one of the following methods to complete the survey. All 1199SEIU Home Care members receiving this letter must complete the survey by July 17, 2023.

Call BeneStream’s toll-free line at (833) 521-1981 Monday through Friday from 8:00 am to 8:00 pm to take the survey over the phone. BeneStream’s telephone representatives are trained to help you complete the survey in just a few minutes and offer translation services in your own language.
Come to the Union at 498 Seventh Avenue, New York, NY and complete the survey with your organizer or Benefit Fund Outreach Coordinators.

Access Care from the Comfort of Your Home
If you are a National Benefit Fund, Greater New York Benefit Fund or Home Care Benefit Fund member, you can schedule a virtual visit with any hospital or provider in our 1199SEIU Benefit Fund network, wherever you may be. This is in addition to your Teladoc benefit.
