Count Me In: Campaigning for the Future of Long Term Care
“The show must go on!” Despite forecasts of a foot of snow and possible school closings, that was the spirit that over 200 nursing home staff expressed on Wednesday, January 12 at the 11th QCC (Quality Care Community) conference. QCC is a collaborative effort of the union’s Nursing Home Division and the employers committed to improve care for residents and staff of the homes. Staff, families and even two nursing home residents traveled from every borough and upstate NY to participate in the conference in Manhattan.
“Count Me In: Campaigning for the Future of Long-Term Care” was the theme this year. Participants were challenged by Arthur Webb, former CEO of Village Care, to embrace the changes facing long-term care. An expert panel of union and management leaders offered their perspectives on the economic and political realities impacting long-term care. 1199SEIU Vice President Pearl Granat argued that “It’s never too late to change,” and that both labor and management need to look to new models and ways of caring for the frail and disabled.
Following a round of informative and creative workshops, the participants undertook activities that demonstrated the importance of compassion, team work, sharing information and timely communication. The activities focused on the new MDS 3.0 form which identifies the specific care that nursing home residents require and impacts directly on reimbursement. Facilities that utilize recommended workforce practices will provide higher quality care and realize increased income.
The conference ended with an address by Dr. Bertice Berry who used humor and personal narrative to encourage the attendees to continue to care and grow. “When you walk with purpose you collide with destiny,” she counseled.
Recreation as a Residents’ Right

This workshop will introduce participants to a grant-funded guide for expanding therapeutic recreation options through the guidance and oversight of a labor-management committee. New survey processes along with the concept of therapeutic recreation as a resident right will be explored; specifically focusing on purpose and the dignity obtained through having meaning in one’s life. The workshop is intended for all disciplines.
Coaching Communication Skills to Promote Culture Change
Karen Blick & Danita Yizer – Schnurmacher Center for Rehabilitation & Nursing
Presenters will demonstrate how they use the core skills of coaching communication to promote successful and positive interaction with staff, colleagues, residents and family members. These skills can be integrated into your facility’s customer service training. This material is based on a curriculum developed by PHI (Paraprofessional Healthcare Institute), a national non-profit organization promoting the link between quality jobs and quality care.
Case Mix Index: CMI Charting Project
Morningside House: Sally Martin, Evelyn Martinez, Sharon Lloyd & Aimee Grant
The workshop will provide best practices to implement a Case Mix Index project. Morningside House piloted the process improvement project to improve efficient documentation that drives reimbursements to the facility. The project incorporated interdisciplinary teams to provide a comprehensive approach to care delivery for residents. The outcome of the project resulted in significant revenues to the facility and labor management cohesiveness.
Person Centered Performance Improvement
Ann Wyatt, Staff from Cobble Hill Health Center & Isabella Geriatric Center
“Getting Better All the Time” is a user-friendly performance improvement manual based on the experiences of Cobble Hill Health Center and the Isabella Geriatric Center. The manual’s steps in the PDSA problem-solving process (Plan, Do, Study, Act) will be explored, with specific examples from 2 facilities. This approach emphasizes the inclusion of all stakeholder perspectives, including direct care staff, managers, residents & families.
Heroic Conversations: A Key to Organizational Success

In this highly interactive session, participants will be introduced to the Heroic Conversations® technique, a proven method for negotiating “difficult conversations” more quickly and effectively. As part of the seminar, participants will be invited to investigate and share where their typical negotiation strategies are falling short. They will then be guided through an innovative technique for dissolving communication barriers and transforming negotiations from frustration and impasse to clearer understanding and agreement.

Essential Elements of the Small House Approach to Person-Directed Care
Households, small houses, greenhouses . . .You have heard the separate terms but what do they all have in common? During this workshop we will identify and discuss the essential elements of the small house approach to delivering person-directed care. Focus will be on the organizational, physical and operational assumptions they share, while differences between the approaches also will be explored. The impact on staff, residents and families will be core throughout the discussion.

Join national expert, Joseph Ouslander, MD, and CCLC leadership in an interactive workshop on Caring Together: INTERACT NY – Interventions to Reduce Preventable Acute Care Transfers. INTERACT NY is a quality improvement initiative designed for all members of the interdisciplinary team. This workshop will introduce evidence-based tools used at different stages of care to aid in the early identification, assessment, documentation, and communication of significant changes in a resident’s status.

The Service Excellence workshop is a template for transitioning staff from a long term care service delivery model to short-term rehab at their new facility. Village Care implemented this approach to enable staff to understand the business case for change to a boutique style, 5-star hotel. The workshop is experiential and engaging; it provides participants with the tools and resources to adapt to a new client base, automation of equipment and a team approach to care practices.
