Introducing Ochsner Digital Medicine

We’re excited to introduce you to Ochsner Digital Medicine, a program to help you take control of your high blood pressure and/or Type 2 diabetes wherever you are—without an extra visit to the doctor’s office. Ochsner Digital Medicine uses technology and data to create a customized health plan to monitor and manage your hypertension and diabetes more effectively.

How does the program work?

Receive a free blood pressure cuff and/or glucometer to take readings using your smartphone or tablet.
Your care team monitors your readings and manages your medications as needed to make sure you’re receiving the right treatment and dose.
Professional health coaching and resources help you build healthy lifestyle habits.

4 out of 5 participants reach their blood pressure or diabetes goals within six months of joining the program.
