Contributing Employer
Any hospital, nursing home, health facility or institution or entity in a related field or any retail drug store which has a collective bargaining agreement by the terms of which it agrees to make contributions to the Pension Fund.
Employer Pension Fund Participation Start Date
The date your Contributing Employer began contributing to the Pension Fund for members of the bargaining unit in which you were employed.
Final Average Salary
The average of your regular wages (not including overtime, bonuses or other additional pay) for the five consecutive years in which you earned the highest regular salary during your last 10 years of covered employment.
First Day Worked
Your first day of employment with a Contributing Employer.
Last Day Worked
Your last day of employment with a Contributing Employer.
Present Weekly Regular Salary
The total weekly pay earned while working for a Contributing Employer, excluding overtime, on-call pay, commissions, bonuses and gratuities.
Requested Pension Start Date
Date you request your pension benefits to begin.
Years of Future Service
The years and months that you worked in a Covered Job Category after your employer started participating in the Plan.
Years of Past Service
The years and months that you worked in a Covered Job Category before your bargaining unit started participating in the Fund.
