Reimbursement for Licensure and Certification Fees
Dietitians: Up to $179 every three (3) years for the New York State licensure fee and up to $60 annually for the Commission on Dietetic Registration (CDR) credentialing fee.
Licensed Practical Nurses (LPNs): Up to $73 every three (3) years for the New York State licensure fee.
Microbiologists, Laboratory Technologists, Laboratory Technicians, Phlebotomy Technicians and Hospital Technicians (Lab Assistants): Up to $196 every three (3) years for the New York State licensure fee.
Pharmacists: Up to $224 every three (3) years for the New York State licensure fee.
In order to receive reimbursement, you must complete a General Enrollment Form and submit it, along with all required proofs, to the Fund via e-mail to [email protected]. Required proofs include:
- A copy of the licensure or certificate; and
- Proof of cost and payment.
The completed form and all required supporting documents must be submitted to the Fund within ninety (90) days after the issue date of the licensure or certificate.
