COVID-19 Vaccines Now Available to Home Care Workers
Home care workers can be vaccinated against COVID-19 at sites throughout New York City and New York State. Most vaccination sites require an appointment, and those getting vaccinated must complete the New York State COVID-19 Vaccine Form, bring proof of employment, such as an employee ID card, a letter from an employer or affiliated organization, or a recent pay stub. For more information and to schedule an appointment, you may visit one of these websites:

If you are a home care worker, you can make an appointment to get vaccinated through either the New York City or New York State website. If you need assistance, your Outreach Coordinator is available to help. In order to further assist you, we will forward your email to them. If you need additional assistance, you can directly email OutreachAssistance@1199Funds.org or find your Outreach Coordinator’s contact information here.
