    Required Documents

    For First-time Applicants

    You must submit the following documents, along with your application, to register:

    • IMPORTANT CHANGE: If you are a member of Wage or Eligibility Class I or II, you do not need to submit your pay stubs anymore. However, you still need to provide your pay stubs if you fall into any of these categories:

      • You work for an employer that does not contribute to the 1199SEIU National Benefit Fund
      • You want to enroll your child in the Child Care Learning Center
      • You make less than $35,000 per year
      • You are receiving short-term disability, workers’ compensation or NYS Paid Family Medical Leave
    • If you are on workers’ compensation, short-term disability or NYS Paid Family Medical Leave, you must submit a copy of your last pay stub and your most recent workers’ compensation, disability or NYS Paid Family Medical Leave stub. If the pay stub is not available, a letter from your employer is required. The employer letter must include the following: member’s name; last four digits of the member’s Social Security number; hourly pay rate; date of hire; and weekly work hours. The expected date of return must be included for members on short-term disability or workers’ compensation.
    • A copy of your 1199SEIU Health Benefits ID Card, listing your dependents between birth and age 17. If your children are not listed on the 1199SEIU Health Benefits ID Card, other acceptable proof of legal dependency is required, such as

      • Adoption papers listing you as guardian
      • Birth certificate listing you as parent
      • A copy of last year’s signed federal tax return listing your adopted and/or biological child(ren)

      If your bargaining unit does not contribute to the 1199SEIU Benefit and Pension Funds, you may submit your participating health insurance ID card, provided that your dependents are listed on it. If not, you must provide proof of legal dependency as listed above.

    • A copy of the Social Security card of each child for whom you are applying. The CCF cannot register any child without a Social Security number/card, including newborn children who do not have one. There are no exceptions to this rule.

    If you do not have a copy of a child’s Social Security card, you must provide the CCF with either of the following:

    • An original letter from the Social Security office, showing the child’s Social Security number; or
    • A copy of last year’s signed federal tax return, showing the child’s Social Security number.

    If you are missing any of the required documents, the CCF cannot register your child(ren).

    For Returning Applicants

    You must submit the following documents, along with your application, to register:

All events are informative, can serve as collaborative tools for you and your family, and are designed to help you better balance the demands of your career with the challenges of parenthood.

  • Program and Member Changes Requiring Written Notification

    Dropping an Approved Program

    You must notify the Child Care Funds (CCF) in writing in order to drop a program for which you have been approved. The CCF will not consider a program officially dropped until a written notice is received. Requests must be made four (4) weeks before a program starts.

    • You are responsible for notifying both the provider and the CCF in writing should you decide to drop a program. If you fail to notify the CCF of your decision to drop a program within the designated time period, you will be held responsible for any co-payments due.
    • If the CCF is not properly notified in writing of your decision within the designated time period, the program will still be reflected on your benefit history, your co-payment may not be refunded and you will be placed at the bottom of the distribution list by Union seniority and benefit history in the following

    Requesting Program Changes

    You must submit change requests to the CCF in writing. The CCF will not process the change request unless written notification is received. Requests for changes may be emailed to Registration@1199Funds.org. Please include your full name and Member ID number.

    • Only one (1) program change is permitted.
    • For programs that require a co-payment, change requests will not be considered if the co-payment is not paid by the deadline. Please refer to page 26 for co-payment due dates.
    • Program changes are only available within the same program type. For example, you may be able to transfer from one summer day camp provider to another summer day camp provider. Changes between two different program types, for example, from a holiday program to a summer day camp, are not permitted.
    • Changes must be requested four (4) weeks before a program starts or before the provider’s stipulated withdrawal deadline, whichever comes first.
    • No change request is guaranteed.
    • All program changes are subject to the availability of monies, program type and individual agreements with the service providers, to accommodate the request.
  • Loss of Approved Benefits

    The following situations can result in a loss of benefits:

    Terminations and Layoffs. All benefits will end 30 days after the date of your termination or layoff.

    Workers’ Compensation and Disability. Members receiving workers’ compensation or short-term disability will continue their benefits for up to a maximum of 26 weeks. After this point, all Child Care Funds (CCF) benefits will immediately end unless the member has returned to work. The CCF will require documentation from your employer confirming your return to work.

    Failure to Make Applicable Co-payments by the Deadline. In order to claim the benefit for which you were approved, you must submit all applicable co-payments by the indicated deadline. If you fail to make your co-payment, your approved benefit will be canceled.

    Failure to Submit Required Reimbursement Forms. If you do not return the required reimbursement forms within five (5) days of the due date, the approved reimbursement will be canceled.

    Position (Title) Changes. If you transfer to a non-Union position or to an 1199SEIU collective bargaining position that does not contribute to the Child Care Funds, your approved benefits will be cancelled within 30 days.

    Employers’ Failure to Make Payments to the Funds. Members will receive a letter from the National Benefit Fund or the Greater New York Benefit Fund informing them of the termination date. All benefits will cease on the termination date indicated in the letter. If your employer makes the necessary payments before the termination date, your benefits will continue without interruption.

    FALC Silver Anniversary: Investing in Our Future, One Child at a Time

    This year, the Child Care Corporation is celebrating 25 years of the Future of America Learning Center (FALC). Since the day it opened on October 3, 1993, FALC has provided nearly 9,000 children of 1199SEIU members and the community with the opportunity to build and enhance their physical, emotional, cognitive and cultural growth. Our program serves children ages 18 months to 5 years, culminating in a Universal Pre-K ceremony each year. This summer, we celebrated 46 students graduating from the program. The ceremony was complete with song and dance performances, well wishes from instructors and heartfelt testimonials from parents about FALC’s nurturing environment.

    FALC also holds other events throughout the year, including an annual Father’s Day breakfast, community outreach activities and educational field trips to museums around New York City. The annual FALC Science Fair is an exciting opportunity for students to take part in science, technology, engineering, arts and math (STEAM) projects. Students are able to learn the value of science while expressing their creativity among peers, teachers and family.

    Students Reach Academic Goals with Joseph Tauber Scholarship Program

    The continual rise of college tuition costs presents challenges for many students looking to affordably continue their education after high school. The Joseph Tauber Scholarship Program (JTS) offers scholarships to children of eligible 1199SEIU members to support undergraduate study at accredited universities. The award—which can help pay for tuition, books and personal expenses—is renewable up to the age of 23.     Degrees received: Bachelor of Arts and Doctor of Medicine from Columbia University

    Andre used his scholarship to pursue his undergraduate degree at Columbia University, and continued on as a medical student. In 2018 he received an MD from the College of Physicians and Surgeons. Andre believes the need-based awards he received during his undergraduate work were essential in funding his college education.

    Katelynne graduated from Newbury College Summa Cum Laude with a degree in Psychology. She was the 2017 recipient of the President’s Award for academic excellence, and was selected to be the student speaker at graduation. Outside of the classroom, Bazile has served as an Executive Admissions Ambassador, Resident Assistant, Orientation Leader and Peer Mentor.     The Children’s Museum of Manhattan is pleased to work with the 1199SEIU Child Care Funds in sharing its “All the Way to K and Beyond!” initiative. “All the Way to K and Beyond!” is an early childhood learning and literacy initiative created in partnership with the NYC Department of Education and the NYC Administration of Children’s Services, and funded by the W.K. Kellogg Foundation and the Meringoff Family Foundation. The initiative provides parents, educators and caregivers with fun, effective, free/low-cost tools and activities to support kindergarten readiness and a love of learning.
