Documents to Be Submitted

End of the school year but no later than Friday, July 15, 2022

2021–2022 Official Transcript, if the student:

Received a 2021–2022 LPN Welfare Fund Scholarship award; and
Maintained full-time status; and
Received a Grade Point Average (GPA) of 3.0 or higher in both the Fall 2021 semester and the Spring 2022 semester or either semester.

Tax Statement

Please note that the Internal Revenue Service (IRS) has determined that an LPN Welfare Fund Study Incentive award is a taxable benefit. If a member’s child is awarded a scholarship, the value of the scholarship must be included on the member’s tax return when filing taxes the following year.
